Keep smiling! Memorial offers two dental plans through Cigna, so you can choose the coverage that fits your needs. Both plans help cover preventive care, basic and major services, and orthodontia for kids and adults.
Your Plan Options
Dental HMO (DHMO)
- Choose from in-network dentists only
- Out-of-network services aren’t covered (you’ll pay the full cost if you go out-of-network)
Dental PPO (DPPO)
- Freedom to see in-network or out-of-network dentists
- Out-of-network care may cost more
- Tip: Ask your dentist to predetermine your claim so you know your costs in advance
What is covered?
- Preventive and diagnostic services: Routine exams, cleanings, fluoride treatments, sealants, and X–rays
- Basic services: Fillings, extractions, root canals, and oral surgery
- Major services: Crowns, dentures, and gum disease treatment
- Orthodontia: Braces, Invisalign, etc. for children and adults
Using Your Dental Benefits
- Contact Cigna Dental customer service at 800-244-6224. You will need to provide your Cigna ID, date of birth , and their home ZIP Code. Your Cigna ID is your employee number preceded by 99. If your employee number is 5 digits long, please add 9900. (example: 990012345 for employee number 12345)
- You may also enroll in the MY Cigna portal by entering your name, address, and Cigna ID. You will then be prompted to create a username and password.
