Keep smiling! Memorial offers two dental plans through Cigna, so you can choose the coverage that fits your needs. Both plans help cover preventive care, basic and major services, and orthodontia for kids and adults.

Your Plan Options

Dental HMO (DHMO)

• Choose from in-network dentists only
• Out-of-network services aren’t covered (you’ll pay the full cost if you go out-of-network)

Dental PPO (DPPO)

• Freedom to see in-network or out-of-network dentists

• Out-of-network care may cost more
• Tip: Ask your dentist to predetermine your claim so you know your costs in advance

What is covered?

• Preventive and diagnostic services: Routine exams, cleanings, fluoride treatments, sealants, and X–rays
• Basic services: Fillings, extractions, root canals, and oral surgery
• Major services: Crowns, dentures, and gum disease treatment
• Orthodontia: Braces, Invisalign, etc. for children and adults

• Provider: Cigna Dental
• Have this ready when you call or schedule: Member ID, Employer Name, and plan name (DHMO or DPPO)
