Eligibility

Are you in a benefits-eligible position and work 40 hours or more per pay period? If you answered yes, you may be eligible to receive benefits after 30 days of employment with Memorial Healthcare System. You can make changes every year during Open Enrollment or throughout year if you have a qualifying life event such as:

  • Marriage or divorce
  • Death of your spouse or dependent
  • Birth or adoption of a child
  • Your spouse terminating or obtaining new employment (that affects eligibility for coverage)
  • You or your spouse switching employment status from full-time to part-time or vice versa (that affects eligibility for coverage)
  • Significant cost or coverage changes
  • our dependent no longer qualifying as an eligible dependent

You must notify and submit applicable forms and/or documentation to Human Resources Shared Services within 30 days of the event, with the exception of documentation for a newborn baby, for which you have 60 days. Human Resources will review your request and determine if the change you are requesting is allowed. Only benefit changes that are consistent with the qualifying life event are permitted.

Your dependents can take advantage of your benefits if they are:

  • Your legal spouse
  • Your child(ren) up to age 26
  • A natural child
  • A stepchild
  • A legally adopted child
  • A child placed for adoption
  • A child placed for foster care
  • A child for whom legal guardianship has been awarded to you or your spouse
  • Unmarried children of any age who become mentally or physically disabled before reaching the age limit

You may be able to enroll your eligible adult child(ren) in a separate medical plan if they are between the ages of 26 and 30, are unmarried, do not have dependents of their own, are a resident of Florida or a student, and do not have other coverage or have a gap in coverage of more than 63 days.
