Memorial offers two different medical plans, giving you the freedom to choose a plan that works best for you. Both plans are administered by Community Care Plan.

Memorial Managed Care Plan (MMCP)

Works like an HMO
You’ll pay set copays for covered services after meeting your annual deductible
In-network only: Services are covered only when you use providers in the Memorial Health Network (and Aetna network if you live in Palm Beach County)
Exceptions: Out-of-network care is covered only in emergencies or when the needed service isn’t available in-network (with prior authorization)

Memorial Consumer Health Plan (MCHP)

A consumer-driven plan with an annual deductible and coinsurance
Offers out-of-network benefits: you can see providers outside the Memorial Health Network (and Aetna network if you live in Palm Beach County) at a higher cost
Gives you more flexibility if you want to keep your current provider

What you need when you call: Member ID, Date of Birth, and Plan Name (MMCP or MCHP) etc.

Finding & Using Your Plan

Find your Member ID: Check physical ID card, or log into Memorial MyChart to find a digital version your card that has your member ID.
Provider directories: See which doctors, hospitals, and specialists are in-network on ccpcares.org.

Can I keep my current doctor?

If your doctor is in the Memorial Health Network → yes!
If your doctor isn’t in-network and you want to keep your current doctor → choose MCHP and pay a higher cost to stay with them
