How to Get Started
To get started with our outpatient behavioral services, you'll need to complete an intake assessment. Assessments are offered by appointment or during our walk-in hours: Monday-Thursday, 8am-5pm.
Your visit will be with a licensed clinical therapist to complete an intake assessment into our clinic.
Please note—an intake assessment is to complete the admission for our clinic, it is not a visit to start medications.
Here are the things you will need to bring with you when you come to the clinic:
- Proof of Identification/residence and health insurance card.
- List of all medications you are currently taking
- If you have a referral please bring a copy with you
- We ask that you do not bring children/minors with you on the time of your visit as staff are not available to supervise them and they cannot be left unattended while you are meeting with staff.
- Please plan to be at the clinic for 2 ½ to 3 hours for your first visit. This length of time is only for the first visit.
