Support Services We Provide
Rebel’s Drop-In Center is a safe space for adults with behavioral health conditions seeking peer support and connection. The center provides free services, including self-help support groups, arts and crafts activities, wellness and recreational activities, peer mentoring and recovery coaching.
Mind/Body Wellness is a fully integrated primary care clinic within our Outpatient Behavioral Health Center. We provide comprehensive medical services that support both physical and emotional health, recognizing that whole person wellness is essential to recovery.
Our clinic offers preventive medicine, on site lab services, specialist referrals, and treatment for chronic conditions such as diabetes and hypertension. The care team is led by a Family Practice Advanced Registered Nurse Practitioner and includes a registered nurse, a medical assistant, and a peer specialist — all working together to deliver coordinated, compassionate care.
In addition to well and sick visits, medication consultation, and chronic disease management, we also provide smoking cessation classes and health education to help patients build lasting habits for healthier living.
As a Certified Community Behavioral Health Clinic (CCBHC) recognized by SAMHSA, our Care Coordination Services are at the heart of our commitment to delivering comprehensive, person-centered, and trauma-informed care. We ensure that every individual receives the support they need across the full spectrum of behavioral, physical, and social health services.
Our Care Coordination team works closely with clients, families, primary care providers, behavioral health specialists, hospitals, social service agencies, and community partners. This integrated approach helps eliminate barriers, reduce duplication of services, and promote whole-person wellness.
Our CCBHC-aligned Care Coordination Services include:
- Comprehensive, culturally appropriate assessments and individualized care planning
- Coordination of services across mental health, substance use, primary care, and specialty care providers
- Referrals and warm handoffs to community-based supports, including housing, employment, education, legal aid, and peer recovery services
- Discharge and transition planning from hospitals, residential treatment, or incarceration.
- Collaboration with criminal justice, child welfare, and veteran systems when needed.
- Crisis planning, safety monitoring, and follow-up support to ensure continuity of care.
Our care coordination services empower individuals and families to take an active role in their recovery, improve health outcomes, and thrive within their communities.
Cost should never be a barrier to care. We accept many healthcare and managed care plans, including Medicare and Medicaid.
Our sliding scale fee program is designed to provide free or discounted care to those with limited or no means to pay for their medical services. We ensure that everyone who seeks healthcare services at Memorial Behavioral Health will be treated, regardless of their ability to pay.
