PGY1 Pharmacy Residency Memorial Regional Hospital South

The Memorial Regional Hospital South (MRHS) PGY1 Pharmacy Residency builds on the Doctor of Pharmacy education and contributes to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions. Post-residency, candidates are eligible for board certification and PGY2 Pharmacy Residency training.

The MRHS Pharmacy Postgraduate Year One (PGY1) Residency is accredited by the American Society of Health System Pharmacists (ASHP). The residents will be integrated into the pharmacy and healthcare team to develop expert knowledge in clinical patient care, medication use systems, and administrative areas. Additionally, the residents will develop skills in leadership, research, project management, and teaching through different longitudinal experiences. The goal of the MRHS program is to develop residents into clinicians and leaders with the knowledge and experience to become confident and integral members of a healthcare team and pharmacy practice.

Application Requirements

Applications will be accepted using PhORCAS. All applicants must register at the Match-ASHP Resident Matching Program and submit an application through the PhORCAS website.

The deadline for applications and all requirements for acceptance can be found on the ASHP Directory. Interested applicants must be graduates of an Accreditation Council for Pharmacy Education-accredited Doctor of Pharmacy degree program, and be eligible for Florida Pharmacist Licensure and a Florida Pharmacy Intern license.

Memorial Regional Hospital South

Memorial Regional Hospital South (MRHS) is a 216-bed facility and is part of the Memorial Healthcare System located in South Florida. In addition to providing exceptional emergency and acute medical care, MRHS is also home to Memorial Rehabilitation Institute, South Florida’s largest provider of physical rehabilitation services.

This facility provides the opportunity to care for a variety of patient populations through a patient- and family-centered, collaborative, and multidisciplinary approach.

About Memorial Regional Hospital South Pharmacy
Our mission is to provide high quality, cost-effective, comprehensive, and safe pharmaceutical services to the Memorial Regional Hospital South staff and patients. The MRHS pharmacists practice in a variety of patient care areas providing decentralized clinical and specialist services throughout our facility. The Pharmacy Department is actively involved in multidisciplinary patient care, as well as facility and district-wide committees. The department embodies an educational environment promoting personal and departmental growth and development.

Faculty
Vishwas Dave
Vishwas Dave, PharmD, MBA

Rotation: Administration and Leadership

Dr. Dave received his Bachelor of Pharmacy degree from Long Island University, New York, and obtained his Doctor of Pharmacy degree from the University of Florida (UF). Dr. Dave has been the director of pharmacy services at MRHS and also serves as the consultant of record. Prior to accepting a Director’s role, his past experience in hospital pharmacy included clinical pharmacist, medication safety coordinator, operations manager, and director of pharmacy at Memorial Hospital Pembroke. Due to his leadership, many new initiatives have been launched, including transitions of care, a pharmacist-managed antibiotic stewardship program, admission medication reconciliation, Transitions of Care, and numerous other programs. Under his leadership, specialists, coordinators, and pharmacists have had the opportunity to publish in local, state, and national-level organizations.

 

Natalie Kerr
Natalie Kerr, PharmD, MBA, BCSCP

Rotation: Administration and Leadership

Dr. Kerr obtained her Doctor of Pharmacy from Larkin University. She also pursued a Master’s in Business Administration from Nova Southeastern University to focus on the business operations of pharmacy services. She has led many initiatives throughout her career, which include managing a 340b program, a Medicaid reimbursement project, and a Meds to Bed program. Dr. Kerr currently holds a Board Certification in Sterile Compounding and leads the USP 797 Taskforce across the Memorial Healthcare System. Her professional affiliations include ASHP and FSHP. Professional interests include pharmacy leadership, medication safety, and advanced hospital practice.

 

Natasha Romero
Natasha Romero, PharmD, BCCCP, BCPS

Rotation: Orientation, Research

Dr. Romero received her Bachelor of Science in Biology from Boston College and her Doctor of Pharmacy from MCPHS University. She went on to complete a PGY1 and PGY2 in Critical Care at Brigham and Women’s Hospital and holds Board Certification in Pharmacotherapy and Critical Care. She currently serves as the Pharmacy Clinical Manager and MRHS PGY1 Residency Director. Her professional interests include critical care, antimicrobial stewardship, anticoagulation management, and clinical research.

 

Stephen Berkowitz
Stephen Berkowitz, PharmD, BCPS

Rotation: Emergency Medicine

Dr. Berkowitz earned a Bachelor of Science in Computer Information Systems in 2004 and completed a Doctor of Pharmacy degree in 2008, both from Nova Southeastern University. He subsequently completed an ASHP-accredited PGY1 residency at Memorial Regional Hospital. He has over 15 years of experience working in critical care, emergency medicine, and rehabilitation. Dr. Berkowitz is a board-certified pharmacotherapy specialist and holds appointments as a clinical assistant professor at the University of Florida, Florida A&M University, and Nova Southeastern University. His current interests include Emergency Medicine and Diabetes Care.

 

Chantrelle Johnson
Chantrelle Johnson, PharmD

Rotation: Rehabilitation Medicine – Neurology and Brain Injury

Dr. Johnson graduated from Florida A&M University with her Doctor of Pharmacy degree. She completed an ASHP-accredited PGY1 Pharmacy Residency at the Southern Arizona VA Healthcare System. With over a decade of experience, her current role includes APPE and resident preceptor for General Medicine, Neurology and Brain Injury. Dr. Johnson enjoys being a certified BLS and ACLS instructor for Memorial Healthcare System. Her professional interests include serving as an active member of ASHP and FSHP and teaching the future of pharmacy.

 

Jenny Mendoza
Jenny Mendoza, PharmD

Rotation: Rehabilitation Medicine – General

Dr. Mendoza graduated from Nova Southeastern University where she obtained her Doctor of Pharmacy. She went on to complete a PGY1 residency at Holy Cross Hospital. Upon completion, she started working at the Miami Cancer Institute at Baptist Health as a clinical pharmacist; subsequently, she worked at the Memorial Cancer Institute as a Clinical Specialist. Currently, she practices as a clinical specialist for inpatient rehab. Her interests include physical medicine and rehabilitation, internal medicine, and oncology.

 

Levi Morris MD
Levi Morris, PharmD, BCPS

Rotation: Internal Medicine

Dr. Morris conferred two Bachelor's degrees from the University of Central Florida; Microbiology and Molecular Biology, and Health Sciences with a minor in Health Service Administration. He received his Doctor of Pharmacy from Nova Southeastern University. He then completed his PGY1 residency at Memorial Regional Hospital. Dr. Morris holds a Board Certification in Pharmacotherapy. He currently serves in the capacity of clinical pharmacy specialist.

 

Gilnou Pamphile
Gilnou Pamphile, PharmD, MS

Rotation: Medication Safety

Dr. Pamphile earned her Bachelor of Science in Microbiology and Cell Science from the University of Florida and her Doctor of Pharmacy from the University of Utah. She completed a PGY1/2 Health-System Pharmacy Administration and Leadership residency at the Cleveland Clinic Main Campus, along with a Master of Science in Pharmacy Leadership from the University of Cincinnati. She currently serves as the Medication Safety and Quality Coordinator at MRHS. Her professional interests include pharmacy leadership, medication safety, drug information, and supply chain management.

 

Enis Lazo de la Vega
Enisdreysis Lazo de la Vega, PharmD, BCPS

Rotation: Longitudinal Staffing

Dr. Lazo de la Vega graduated from Nova Southeastern University College of Pharmacy with a Doctor of Pharmacy degree. She completed a PGY1 Residency at Memorial Regional Hospital South and holds Board Certification in Pharmacotherapy. She currently practices as a clinical pharmacist and offers an Advanced Hospital APPE rotation. Her professional interests include emergency medicine, antimicrobial stewardship, and internal medicine.
