What is Phase II Cardiac Rehabilitation?

American Association of Cardiovascular and Pulmonary Rehabilitation certification seal

Rehabilitation is an outpatient medically supervised program aimed to improve your cardiovascular health if you have experienced a heart attack, a heart condition, heart surgery or angioplasty.

At Memorial, we believe undergoing a heart event does not have to stop you from living your life to the fullest. Our outpatient cardiac rehabilitation program is designed to rehabilitate your heart, prevent any further heart injuries and give you your life back.

Our comprehensive approach emphasizes:

  • Patient- and family-centered goal setting
  • Heart disease management education
  • Exercise prescription and therapy
  • Nutrition counseling
  • Stress management
  • Risk factor modification, such as blood sugars, cholesterol levels, blood pressure and/or body weight management
  • Tobacco cessation

Our outpatient cardiac rehabilitation program is certified by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) a multidisciplinary organization dedicated to the mission of reducing morbidity, mortality and disability from cardiovascular and pulmonary disease through education, prevention, rehabilitation, research and disease management.
