Our Behavioral Health Transition of Care services at Memorial Regional Hospital are specifically designed individuals with behavioral health conditions who have been treated either in the emergency department or as hospital inpatients.

What services are provided?

Behavioral Health Transition of Care services are designed to reduce readmissions and adverse events by providing patient education, support and linking from one level of care to another. Our multidisciplinary team includes a clinical pharmacy specialist, a licensed clinical social worker, and a peer support specialist that provides patient and caregiver education, assessment, care coordination and discharge planning. They work collaboratively with the emergency department, inpatient and outpatient staff, as well as community-based providers to assure a successful transition for behavioral health patients.
