At Memorial Primary Care, we take ownership of your health alongside you. Your primary care provider brings together all aspects of your healthcare, including specialists, pharmacy, mental health, and social supports, in one medical home.

Whatever your needs, we’re here to help you heal and give you the tools to maintain your health. You won’t feel rushed or like you’re only a number here. Your doctor will take the time to sit down with you, explain things and allow you to ask questions.

Your Healthcare Home

Each of our Memorial Primary Care Centers is designated a Level 3 Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance. What does that mean for you?

Having a nationally recognized medical home means that:

  • You receive preventive care and screenings more often than you would at a traditional practice.
  • We help you proactively manage chronic health conditions.
  • We can handle tasks like referrals, care plans, prescriptions and more during your visit.
  • Our primary care practices meet the highest national standards for quality and patient-centered care.

Our primary care providers work with specialists and refer you to them when necessary. But we try to keep most of your care within our office. We do it for your convenience and, more importantly, to ensure you have one team that stays on top of your health.

Personal Health Coach

Our primary care services include a health coach to assist patients on a case-by-case basis. These team members encourage and educate you. They help with disease prevention, coordinate care for chronic disease management, and reach out after an emergency room visit.

Like everyone on our team, health coaches are respectful of cultural differences. Your health coach will honor your culture and background while helping you get and stay healthy.
