Memorial Outpatient Scheduling
October 2005 — Now you can schedule an outpatient appointment for you or a family member 24 hours a day, 7 days a week. As part of its website, Memorial Healthcare System has created an online appointment scheduling form, which enables you to request preferred dates and times for certain outpatient medical procedures, diagnostic tests (such as X-rays or MRIs) or rehabilitation sessions. A professional from Memorial’s Centralized Scheduling Department will work with you by phone or e-mail to schedule your outpatient appointment.
From our home page, simply click on the “Outpatient Scheduling” link on the navigation menu to your left. Fill out and submit the online form, being certain to provide all required information. Finally, fax a copy of your doctor’s prescription to Centralized Scheduling at (954) 265-4759. A staff member will contact you no later than one business day after receiving your information.
