Health Intervention with Targeted Services (HITS)
This program's superb achievements in improving the health of the uninsured and underserved earned the Sapphire Award from The Blue Foundation for a Healthy Florida, the philanthropic affiliate of Blue Cross and Blue Shield of Florida. HITS is designed to improve healthcare and provide long-term solutions for those in need of assistance.
Key to the program's success is door-to-door visits in targeted neighborhoods to conduct outreach and determine eligibility. HITS helps the uninsured and underinsured in our community establish a "medical home" with South Broward Community Health Services, encourages those with chronic health conditions to participate in our disease management programs, and helps them enroll in such government-sponsored programs as Medicaid and Medicare if they are eligible. The goal of this program is to ensure that 100 percent of program participants are assessed for health insurance coverage and understand the importance of establishing a medical home.
Since its inception in 2006, the HITS program has improved the lives of 12,000 persons in the South Broward County community and linked 1,970 uninsured individuals to a medical home.
